Malaysia Restricted Passport to Singapore (1993 — 1998)

Based on information on page 64, the holder has previously hold a Restricted Malaysian Passport issued in 1974.

Also on the same page, it was recorded by the immigration of Singapore at Woodlands that it was a day trip visit to the island-state on 23 April 1993.

This document is issued solely to facilitate the movement of the bearer to and from Singapore and is not valid for travel to and from places other than Singapore.

Ini adalah satu dokumen yang berharga dan tidaklah boleh diubah dengan apa cara pun, dan juga tidak boleh dibiarkan jatuh ke tangan seseorang yang tidak berkuasa.

Jika hilang atau binasa, pembawanya hendaklah melaporkan kenyataan dan hal-hal mengenai kehilangan atau kebinasaan itu kepada Pejabat Imigresen yang hampir sekali di Malaysia dan membuat satu laporan kepada Polis tempatan.

Permohonan untuk mendapat gantian boleh dibuat kepada Pejabat Imigresen di mana kehilangan atau kebinasaan itu dilaporkan; tetapi gantian bagi sesuatu PASPORT TERHAD MALAYSIA yang hilang akan diberi hanya selepas siasatan yang sepenuhnya dijalankan.

Dokumen ini tidak boleh dihantar keluar Malaysia dengan jalan pos.

Dokumen ini boleh dibaharui di mana-mana Pejabat Imigresen dalam Malaysia Barat.


This is a valuable document and must not be altered in any way, nor should it be allowed to fall into the hands of an unauthorised person.

If it is lost or destroyed, the bearer should report the fact and circumstances of such loss or destruction to the nearest Immigration Office in Malaysia, and lodge a report with the local Police.

An application for a replacement may be made to the Immigration Office where the loss or destruction is reported; but replacement of a lost RESTRICTED MALAYSIA PASSPORT will only be made after exhaustive inquiries.

This document should not be sent out of Malaysia by post.

This document may be renewed at any Immigration Office in the States of Malaya.
